Abstract

Drug interaction was one of the Drug Related Problems (DRPs) that could affect treatment outcomes of patients. Diabetes mellitus with hypertension was a strong risk factor for morbidity and mortality of patients with diabetes mellitus. Appropriate therapy for the management of blood pressure was needed to reduce the risk of increased mortality, slowing diabetic. The purpose of this study was to determine the incidence of drug interactions and to describe the pattern of interaction mechanisms as well as the types of medications that could cause interactions on DM with hypertension inpatients in RSUD Jombang at 2015. This study was the descriptive design. Data were obtained retrospectively. Data collection was performed by taking all the data that meets the criteria of the existing research on the medical records of hospitalized patients with diabetes mellitus with hypertension in RSUD Jombang at 2015. Data were analyzed descriptively based on the mechanism of interaction and the level of significance. The results showed that there were 62.50% (25 patients) of 40 diabetic patients with hypertension potentially experiencing drug interactions. Antidiabetic drugs most widely used in diabetic patients with hypertension was a class of sulfonyl urea 35.00% (14 patients) whereas anti-hypertensive drugs were the most widely used class of calcium canal blocker 50% (20 patients). The most common drug interaction through the mechanism of pharmacodynamic drug interactions, as much as 56.52% (26 cases). Based on the level of significance, drug interaction was most common at the level of unknown significance that was 82.61% (38 cases), the significance 2 as much as 8.7% (4 cases), the significance of four as much as 6.52% (3 cases) and significance 5 as much as 2.17% (1 case). The types of drugs that often interact was insulin aspart and ciprofloxacin. Keywords: Drug interactions, DM with hypertension, Hospital Jombang
