Abstract
Stroke is the leading cause of death in worldwide. Each year approximately 0.2% of the population having a stroke by a third die within the next 12 months, another third regain their independence. Effective clinical pharmacy services to reduce health care cost, and effective in improving the quality of health care. This study was conducted to determine Drug Related Problems (DRPs), that occurs in ischemic stroke patient including drug without indication, indication need drug, improper drug selection, dosage less, more doses, and drug interactions. The method used is the method of cross- sectional and retrospective approach. The population of study were all patient who underwent ischemic stroke therapy in DR. Moewardi Hospital Surakarta start from January 1st TO December 31th, 2014. The samples was patients who underwent ischemic stroke therapy with or without concomitant diseases in DR. Moewardi Hospital Surakarta starting from 1 January to 31 December 2014 that include in inclusion criteria. The results showed that from 97 patients, which had DRPs as 74 patients. The most incidence rate of DRPs was indication need drug as 63,51%, drug interaction as 6,76 %, too low dose as 2,70 %, too high dose as 5,41 % and improper drug as 12,16 %, as well as drug without indication as 9,46 %.
