Abstract
Hypertension is one of the risk factors for cardiovascular disease and kidney disease with an increasing prevalence over the years. Long-term drugs and multiple drug combinations are at risk for Drug Related Problems (DRPs) which are undesirable events in the management of the disease, which may affect the outcome of therapy in patients. Research conducted to find out (DRPs) that occur in patients with hypertension in chronic kidney disease. The research method used is descriptive research that is nonexperimental and retrospective data. The sample of this study were all patients hypertensive with CKD listed in the medical record at patient of internal medicine ward RSUD Ir. Soekarno Sukoharjo in 2017. The sample used is medical record data of patients with diagnosis of hypertension with CKD from January to December of 2017 in accordance with inclusion criteria. The results showed that of the 46 patients, use of antihypertensive monotherapy of 21.73%, antihypertensive combination of two drugs by 39.13%, three-drug combination antihypertensive of 19.57%,Iantihypertensive combination of 4 drugs by 19.56%. Incidence of DRPs was an overdose of 11 patients (23.91%), inaccurate drug selection of 3 patients (6.52%), too low doses 0%. Keywords: Hypertension, CKD, DRPs
