Abstract

Typhoid fever is a systemic infectious disease that is acute in nature induced by Salmonella typhi. Chloramphenicol is the most known drug used in typhoid fever treatment. Antibiotic class including penicillin, cephalosphorine, and fluoroquinolone was also effective for typhoid fever. In addition to antibiotic, the treatment for typhoid fever should be added with supporting medicine such as symptomatic drugs antipyretic, corticosteroid, vitamin, antiemetic used to accelerate healing. The efficiency and effectiveness of drug and cost use is the factor requiring attention. The objective of research was to analyze the mean total cost and to find out the effectiveness of typhoid fever in Inpatient Ward of Wonogiri Marga Husada Hospital in 2013 based on the calculation using Cost Effectiveness Analysis (CEA). The population used was patients with typhoid fever meeting the inclusive criteria. The sampling technique used was purposive sampling one. The cost calculated was the direct medical one consisting of medication, medical service, laboratory, and treatment class costs. The data analysis was conducted using t-test and Chi-Square independency test. The mean total direct medical costs of typhoid fever therapy in askeskin (poor health insurance) and common patients were IDR 4,491,240.01 and IDR 5,603,032.26. The proportion of patient achieving therapy target with amoxicillin antibiotic was 66.67%, chloramphenicol 42.86%, thiamphenicol 66.67%, levofloxacin 71.43%, cephtriaxone 66.67%, and cephotaxim 71.43%. Thiamphenicol antibiotic had the highest effectiveness value with lowest Average Cost Effectiveness Ratio (ACER) score of IDR 802,268.52.
