Abstract

The National Health Insurance (Jaminan Kesehatan Nasional = JKN) is the one using Indonesia Case Base Group (INA-CBG’s) tariff package system currently followed by all hospitals in Indonesia. The tariff decided by INA-CBG’s sometimes was not consistent with the real cost the hospital spends so that a difference occurs. In 2012, the acute lymphoblastic leukemia (LLA) occupies the ninth rank out of ten cases absorbing the largest cost for jamkesmas inpatients in Sanglah Central General Hospital of Denpasar (RSUP Sanglah Denpasar). This research identified the difference of real cost from INA-CBG’s package tariff, analyzed the factors affecting the real cost, and find out the effect of chemotherapy cycle on the cost in LLA patients in RSUP Sanglah Denpasar during January-August 2014. This study was an analytical observational research by collecting data of claim document and medical record of LLA chemotherapy inpatient in RSUP Sanglah Denpasar using purposive sampling technique retrospectively with cross sectional design according to the hospital’s perspective with 114 episodes from 31 patients as the sample. The analysis used was one sample t-test to find out the difference of real cost from INA-CBG’s tariff as well as multivariate analysis to find out the factors affecting the real cost. The result showed that there was a positive difference of cost of IDR 627,904,15, with significant difference between real cost and INA-CBG’s tariff (p value = 0.000). The length of Stay (LOS), main diagnosis, secondary diagnosis and procedure was the factor affecting significantly the real cost in which the four factors having p value = 0.000. The patients with suitable chemotherapy cycle interval had lower cost than those in unsuitable chemotherapy cycle interval. INACBG’s package cost was the one sufficient for meeting the cost needed for treating the LLA JKN chemotherapy inpatient in 2014. Key words : LLA, JKN, Cost, Chemotherapy, INA-CBG
