Abstract
In the implementation of national health insurance, the payment pattern in advanced facility level based on INA-CBGs tariff, considering the prevalence and treatment cost of type 2 diabetes mellitus disease allows the difference of INACBGs tariff from the real cost of patient treatment. The objectives of research were to find out the representation of drug use pattern, to find out the cost component with largest contribution to real cost, and to find out whether or not there was an effect of secondary diagnosis on real cost in the national health insurance inpatients with type 2 diabetes mellitus in RSU Anutapura Palu (Anutapura General Hospital of Palu). This study was an observational analytical research according to hospital perspective, with retrospective data collection method to collect data from national health insurance claim documents during September 2014-February 2015 with code INA-CBGs E-4-10-I, E-4-10-II and E-4-10-III. Data analysis was carried out using descriptive analysis technique to find out the patient characteristic and representation of drug use pattern, one sample t test to compare the real cost with INA CBGs tariff and correlational test to find out the effect of secondary diagnosis on total real cost. The result of research showed that the most commonly used drug was biguanide in the first severity level, combination of biguanide and sulfonylurea in the second severity level, and combination of biguanide and insulin in the third severity level. In each of severity level, there was a positive difference of real cost from INA CBG tariff of IDR 112,691,704 for code E-4-10-I, of IDR 37,534,965 for code E-4-10-II, and of IDR 8,159,424 for code E-4-10-III. The component cost with the largest contribution to the real cost was accommodation cost and drug cost. Secondary diagnosis was affected significantly the real cost.
