Abstract

The National Health Insurance (Jaminan Kesehatan Nasional = JKN) applies the INA-CBG’s tariff constituting the package so that the high real cost and INA-CBGs package tariff can result in difference. The objective of research was to find out the difference of real cost without chemotherapy and INA-CBG’s package tariff, to analyze the factors affecting the real cost without chemotherapy, and to find out the amount of real cost with chemotherapy related to chemotherapy cycle regularity in breast cancer chemotherapy patient in RSUP Sanglah from January to July 2014. This research employed a descriptive analytical design by collecting the data of claim document and medical records of breast cancer chemotherapy inpatient in RSUP Sanglah (Sanglah Central General Hospital of Denpasar) taken using purposive sampling retrospectively with cross-sectional design according to the hospital’s perspective; the sample consisted of 383 episodes from 117 patients. The analysis was carried out using one sample t-test to find out the difference of real cost and INA-CBG tariff and multivariate analysis to find out the factors affecting the real cost. The result of research showed a significant difference (p < 0.05) between INA-CBGs package tariff and real cost without chemotherapy in the first severity level of IDR 237,768,368 (n=58) for the first, IDR 40,888,130 (n=11) for the second, and IDR 522,010,986 (n=173) for the third classes of treatment; in the second severity level of IDR 388,705,874 (n=47) for the first, IDR 97,195,907 (n=13) for the second, and IDR 301,217,583 (n=47) for the third classes of treatment, and in the third severity level of IDR 74,148,926 (n=1) for the first, IDR 9,127,349 (n=7) for the second, and IDR 199,512,556 (n=26) for the third classes of treatment. The factors affecting the real cost without chemotherapy (p < 0.05) in the first severity were LOS (p = 0.015), procedure (p = 0.000), and treatment class (p = 0.000). Those in the second severity were LOS (p = 0.049), secondary diagnosis (p = 0.002), procedure (p = 0.000) and treatment class (p = 0.000). Those in the third severity were age (p = 0.012), LOS (p = 0.001), procedure (p = 0.028), and treatment class (p = 0.050). The mean real cost without therapy in irregular chemotherapy cycle (IDR 4,409,406) showed higher value than that in regular chemotherapy cycle (IDR 1,781,827).
