Abstract

In Indonesia, respiratory disease i.e Chronic Obstructive Pulmonary Disease (COPD) and Pneumonia are the leading cause of illness and deaths. Both of these diseases also cause significant economic burden for society. But with the Health Insurance in Indonesian, society gets sustainable health care financing and cost required to be charged to insurer. The purpose of this study was to determine total of economic burden of COPD and pneumonia therapy for outpatient and inpatient and determine correlation of patient characteristic to treatment cost of COPD and pneumonia. This study was observational by cross-sectional of health insurance patient with COPD and Pneumonia in Public Services Agency of Konawe Hospital in January-December 2014. Data collection were retrospectively through medical record. Data analysis starts with patient characteristic then followed by cost treatment analysis of COPD and pneumonia using top-down approach. Correlation test was conducted using Pearson correlation. The results showed that therapy cost per year for outpatient with COPD in 101 patients was Rp. 12,553,047, - and for inpatient in 76 patients was Rp. 76,625,025, - while therapy cost per year for outpatients with pneumonia in 42 patients was Rp. 6,703,428, - and for inpatient in 98 patients was Rp. 88,603,876, -. Factors that influence treatment cost of COPD inpatient were age (p=0.022) and duration (p=0.000). Factors that influence the treatment cost of pneumonia outpatient was secondary diagnosis (p=0.023). While on inpatient were age (p=0.000), secondary diagnosis (p=0.007), treatment class (p=0.015) and lenght of stay (0,000). Keywords: cost of illness, cost analysis, COPD, pneumonia.
