Abstract

Heart failure is one of the most prevalent diseases and also the number one cause of death in the world. The number of deaths that occurred in Indonesia made the prevalence of heart failure continues to increase at this time. Cost of illness in cases of heart failure as very expensive. Ernment implement the National Health Insurance in 2014 for public health with the implementation of the payment system of health facilities INA-CBGs rates, which is expected to be a solution in controlling health care costs. This study aims to determine how big the difference between the real cost at the rate of INA-CBG's, factors affecting the real cost, and appropriateness of treatment. This study was an observational analytic study, retrospective data retrieved from the patient files a claim period from January – May 2014. Data analyzed using one - sample t test to compare the average cost of the real rate of INACBG's, multivariate correlation test to determine the factors that affect the real cost, and appropriateness of therapy by looking at the use of the drug for patients hospitalized with diagnosis. The results showed a positive difference between the real cost of the package fare INA-CBG's, to code the I-4-12-I, grade 1 rate is Rp 35,655,408, grade 3 treatment is Rp. 31,005,468. For code INA-CBG's I-4-12-II, class 1 treatment is Rp 80,538,037, class 2 treatment is Rp. 32,758,009, and grade 3 treatment is Rp. 78.30829 million. For code INA-CBG's I-4-12-III, class 1 treatment is Rp. 39,277,555, and grade 3 treatment is Rp. 42.23206 million. The factors that affect the real cost of treatment of patients, namely LOS, procedures and severity. The results of the analysis of the suitability of the drug contained 59 episodes and 5 episodes of care appropriate care is not appropriate treatment of 64 episodes.
