Abstract
The National Health Insurance (Jaminan Kesehatan Nasional = JKN) is the one using Indonesia Case Base Group (INA-CBG’s) tariff package system. The objective of research was to find out the difference of real cost without chemotherapy and INA-CBGs tariff, the factors affecting the real cost, and the amount of chemotherapy real cost related to the chemotherapy cycle regularity in cervical cancer inpatient in Sanglah Central General Hospital of Denpasar (RSUP Sanglah Denpasar) in January-July 2014. This study employed a descriptive analytical design by collecting the data of claim document and medical records of cervical cancer chemotherapy inpatients in RSUP Sanglah (Sanglah Central General Hospital of Denpasar) taken using purposive sampling retrospectively with cross-sectional design according to the hospital’s perspective; the sample consisted of 161 episodes from 50 patients. The analysis was carried out using one sample t-test to find out the difference of real cost and INA-CBG tariff and multivariate analysis to find out the factors affecting the real cost. The difference between real cost and INA-CBGs package tariff in the first severity level of IDR 3,667,671 (n=21) for the first, IDR 3,720,425 (n=11) for the second, and IDR 3,226,915 (n=77) for the third class of treatment. The difference in the second severity level was IDR 7,366,832 (n=4) for the first, IDR 7,364,654 (n=8) for the second, and IDR 6,306,590 (n=27) for the third class of treatment. The difference of cost in the third severity level of IDR 10,451,747 (n=2) for the first, IDR 9,795,986 (n=2) for the second, and IDR 9,058,462 (n=8) for the third classes of treatment. The factors affecting the real cost in the first severity were LOS (p = 0.000), procedure (p = 0.011), and treatment class (p = 0.000). Those in the second severity were LOS (p = 0.000), procedure (p = 0.000), and treatment class (p = 0.001). Those in the third severity were LOS (p = 0.009), procedure (p = 0.039), and treatment class (p = 0.002). The irregular cycle pattern had average real cost without chemotherapy higher than the regular one. Keywords: Cervical Cancer, JKN, Cost, Chemotherapy, INA-CBG’s.
