Abstract

Implementation of Jamkesmas 2010 constraints about terms of payment, it is possible to occur on patients with heart failure because this is require quite high costs. This study aims to determine how big the difference between the real cost at the rate of INA-CBG's, factors affecting the real cost, and appropriateness of treatment patient Jamkesmas hospitalized heart failure in RSUP Dr. Sardjito. This study was an observational analytic study, retrospective data retrieved from the patient files a claim period from January-October 2013. Data analyzed using one- sample t test to compare the average cost of the real rate of INA- CBG's, multivariate correlation test to determine the factors that affect the real cost, and appropriateness of therapy by looking at the use of the drug for patients hospitalized with diagnosis. The results showed a large difference between the real cost of the package fare INA-CBG's I-4-12-i with rates in 2013 amounted Rp.12.702.346 with rates for Rp.23.301.898 2014 ; I-4-12-ii with rates for Rp.175.399.763 2013, with the 2014 rates for Rp.267.121.343 ; I-4-12-iii with rates for Rp.125.791.085 2013, with rates in 2014 amounted Rp.218.016.505. The factors that affect the real cost is the severity of the procedure, secondary diagnosis, LOS, age, and gender. The results of the analysis of the suitability of the drug contained 84 episodes and 12 episodes of care appropriate care is not appropriate treatment of 96 episodes.
