Abstract

Baubau Secondary Hospital as a referral hospital for its surrounding area continually trying to improve the quality of health services, but limitations of equipment, facilities and infrastructure, and human resources, become obstacles. One of them is still apply rooming system at the Intensive Care Unit (ICU), consequently potential to cause medication error. The purpose of this study was to determine the incident of medication error in ICU patients at Baubau Secondary Hospital and the factors that influence medication error. This research was a descriptive research against 32 patients in the ICU during November 2013 to January 2014. Data was collected using questionnaire that was tested with Delphi method, check list observation sheet, data collection sheet, and focus group discussion guideline. Analysis of data performed by descriptive percentage. The results showed that the largest number of medication errors was administration errors with 144 errors (46.91%), then dispensing errors with 119 errors (38.76%), and the smallest was prescribing errors with 44 errors (14.33%). Largest error of administration errors was noncompliance with aseptic method, dispensing errors was the error in writing copy of prescription, whereas prescribing errors was prescription drugs at low doses.
