Abstract

Safe use of the drug is one of the main components in patient safety but medication errors still occur and often go undetected. Medication errors can result in patient morbidity and even cause death, so the need to further streng then the system of monitoring mechanisms and make recommendations for corrective actions when errors occur and then reported. This study aims to determine the stage of medication errors with the highest incidence and the factors that cause the occurrence of medication errors in patients Intern at RSD dr. Soebandi Jember. This research was descriptive research against 30 in patients Intern RSD dr. Soebandi Jember during November 2013 to Desember 2013. Data was collected using questionnaire that was tested with Delphi method, check list observation sheet, data collection sheet and focus group discussion guideline. Analysis of data performed by descriptive percentage. The results showed that the incidence of medication errors occur at the highest stage of prescribing error are 199 events (82.23%), followed by administration phase error are 38 events (15.70%), the next lowest incidence of dispensing errors at this stage are 5 events (2.07%) with indicators that play a role in it is the interaction of the drug, completeness recipes, stands, taking drugs, drug compounding, drug administration, drug delivery forgotten, aseptic methods and attitudes of patients. The factors causing medication error occurrence is a matter of the system (the amount of labor and completeness of the facilities) and professional issues (failure to adopt the policy and procedure documents, the lack of knowledge about the treatment and lack of communication with prescribers).
