Abstract

Drug interactions in patients DM type 2 with complications of hypertension can cause uncontrolled blood glucose levels and blood pressure which affect morbidity, mortality and quality of life of patients. Mortality patients DM type 2 is two times higher due to the complications of DM which include cardiovascular disease, retinopathy, nephropathy, and DM neuropathy. The incidence of drug interactions will increase with the use of more drugs from various complications of the disease, so that when the effectiveness of the drug decreases, it will have an impact on the attainment of blood glucose levels and blood pressure. This research is a non-experimental descriptive study with retrospective data collection using purposive sampling method. The inclusion criteria included medical records of patients diagnosed with DM type 2 with hypertension complications at the Ngudi Waluyo Blitar Hospital inpatient in 2019, patients aged> 18 years, undergoing hospitalization ≥3 days. Exclusion criteria included damaged or illegible medical record data and incomplete medical record data. Based on the inclusion and exclusion criteria, 41 patients met the criteria. To find out the potential drug interactions, use the application Medscape reference-drug interaction checker, Lexicomp, and Stockley's Drugs Interaction Handbook. The data obtained were analyzed using descriptive and bivariate analysis methods. The results showed that there was a potential for drug interactions in patients DM type 2 with hypertension complications in the Ngudi Waluyo Wlingi Hospital in Blitar in 2019 including, pharmacodynamic interactions as many as 90 cases (77.58%), pharmacokinetic interactions as many as 10 cases (8.62%) ) with a minor severity of 19 cases (16.37%), a moderate severity of 95 cases (81.89%), a major severity of 2 cases (1.72%) and no relationship between therapeutic outcome and potential drug interactions. The most widely used therapy is Levemir insulin monotherapy 17.07% for antidiabetic and Amlodipine 34.15% monotherapy for antihypertensive.
