Abstract

D-dimer merupakan biomarker pembentukan dan degradasi fibrin yang dapat diukur dalam darah utuh atau plasma. Kadar D-dimer dapat digunakan sebagai indikator pada infeksi COVID-19. Penelitian ini bertujuan untuk mengetahui korelasi positif kadar D-dimer dengan derajat keparahan pada pasien COVID-19 di RSDM Surakarta. Penelitian ini observasional analitik menggunakan cross sectional study. Sampel yang digunakan adalah pasien COVID-19 berjumlah 60 orang yang dibagi menjadi 3 kelompok yaitu derajat keparahan sedang, berat dan kritis. Teknik pengumpulkan data menggunakan metode puposive sampling dengan penetapan kuota melalui data rekam medik dan data hasil pemeriksaan laboratorium pasien COVID-19 di Rumah Sakit Umum Daerah Dr. Moewardi Surakarta. Penelitian ini dilakukan pada bulan Maret 2021 – Juni 2021 dengan data yang diperoleh pada bulan Maret 2020 – Desember 2020. Uji yang digunakan yaitu uji korelasi Spearman, uji Kruskal Wallis, dan uji Post Hoc. Hasil penelitian menunjukan pada uji korelasi Spearman terdapat korelasi antara kadar D-dimer dengan derajat keparahan pada pasien COVID-19 yang signifikan p=0.010(<0.05), sedangkan nilai r= 0.332 (0.26 - 0.50) dengan kekuatan korelasi “lemah” arah korelasinya adalah positif, berarti semakin tinggi kadar D-dimer pada pasien COVID-19 maka semakin tinggi terjadinya keparahan pada pasien. Hasil uji Kruskal Wallis terdapat perbedaan antara kadar D-dimer dengan berbagai derajat keparahan sedang, berat, kritis pada pasien COVID-19, sedang uji Post Hoc Man Whitney terdapat perbedaan signifikan antara derajat keparahan sedang-berat (p=0.001), sedang-kritis (p=0.015) dan tidak terdapat perbedaan signifikan anatra kadar D-dimer dengan derajat keparahan berat-kritis (p=0.625). Simpulan penelitian ini terdapat korelasi yang signifikan antara kadar D-dimer dengan derajat keparahan pada pasien COVID-19 di Rumah Sakit Dr. Moewardi Surakarta. D-dimer levels can be used as an indicator of COVID-19infection. This study aims to determine the positive correlation of D-dimer levels with severity in COVID-19 patients in RSDM Surakarta. The study was analytically observational using cross sectional studies. The samples used were COVID-19 patients numbering 60 people divided into 3 groups, namely moderate, severe and critical severity. Data collection techniques using puposive sampling method with quota determination through medical record data and laboratory examination data of COVID-19 patients at Dr. Moewardi Surakarta Regional General Hospital. This study was conducted in March 2021 – June 2021 with data obtained in March 2020 – December 2020. The tests used are spearman correlation test, Kruskal Wallistest, and Post Hoc test. The results showed in Spearman's correlation test there was a correlation between D-dimer levels and severity in COVID-19 patients which was significant p=0.010(<0.05), while the value r= 0.332 (0.26 - 0.50) with the strength of the correlation "weak" the correlation direction was positive, means that the higher the D-dimer level in COVID-19 patients, the higher the severity of the patient. Kruskal Wallis test results there is a difference between D-dimer levels with varying degrees of severity of moderate, severe, critical in PATIENTS COVID-19, while post hoc test Man Whitney there is a significant difference between the severity of moderate-severe ( p=0.001), medium-critical ( p=0.015) and there is no significant difference anatra D-dimer levels with the degree of severity of severe-critical (p =0.625). The conclusion of this study is that there is a significant correlation between D-dimer levels and severity in COVID-19 patients at Dr. Moewardi Surakarta Hospital.
