Abstract

COVID-19 merupakan infeksi sistemik yang berpengaruh pada sistem pembekuan darah yang bermanifestasi menyebabkan penyumbatan pembuluh darah. Gangguan koagulasi awal yaitu adanya aktivasi faktor koagulasi intrinsik dan ekstrinsik. Selanjutnya pada fase akhir adanya gangguan penghancuran fibrin dengan penanda terkait fibrin yaitu D-dimer. Tujuan penelitian ini adalah untuk mengetahui hubungan pemeriksaan koagulasi (PT, aPTT, dan D-dimer) terhadap tingat keparahan pasien COVID-19. Sampel penelitian adalah 100 pasien dengan teknik purposive sampling kriteria usia minimal 30 tahun dan terkonfirmasi COVID-19. Penelitian ini termasuk penelitian observasional pendekakatan cross sectional. Data diperoleh dari data sekunder pasien yaitu data rekam medis pasien di RS PKU Muhammadiyah Sukoharjo. Analisis statistik yang digunakan adalah regresi logistik multinominal dengan software komputer, bermakna bila p< 0,05. Hasil penelitian didapatkan pada pemeriksaan koagulasi yaitu pemeriksaan PT (0,166 > 0,05) dan aPTT (0,782 > 0,05), keduanya tidak ada hubungan yang signifikan dengan tingkat keparahan pasien COVID-19. Sedangkan pada pemeriksaan D-dimer (0,000 < 0,05) berhubungan secara signifikan dengan tingkat keparahan pasien COVID-19. Perlu penelitian lanjutan dengan penambahan variabel pemeriksaan terkait koagulasi darah untuk dapat mengendalikan faktor yang mempengaruhi hasil. Kata kunci : pasien COVID-19, PT, aPTT, D-dimer, tingkat keparahan COVID-19 is a systemic infection that affects the blood clotting system which manifests as blockage of blood vessels. Early coagulation disorders are the activation of intrinsic and extrinsic coagulation factors. Furthermore, in the final phase of COVID-19 infection, there is impaired fibrin breakdown, which can be marked by an increase in fibrin-related markers, namely D-dimer. The objective was to determine the relationship between coagulation tests (PT, aPTT, and D-dimer) on the severity of COVID-19 patients. The research sample was 100 patients and use purposive sampling technique with criteria minimun in age 30 and confirmed COVID-19. This research is an observational study with a cross sectional approach. The data was obtained from secondary data, viz. laboratory data and patient medical records at PKU Muhammadiyah Sukoharjo Hospital. Statistical analysis used was multinominal logistic regression with SPSS 21 software, it was significant if p < 0.05. The results of the study were obtained on coagulation examinations, that are PT (0.166 > 0.05) and aPTT (0.782 > 0.05), both of the examinations had no significant relationship to the severity rate of COVID-19 patients. While the D-dimer examination (0.000 < 0.05) was significantly related to the severity rate of COVID-19 patients. Further research is needed with the addition of examination variables related to blood coagulation to be able to control the factors that affect the results.
