Abstract

Diabetes mellitus is a chronic condition that occurs when there is an increase in blood glucose levels because the body cannot produce the hormone insulin or use insulin effectively. The higher prevalence of diabetes causes the cost of health care to rise. The purpose of this study was to test the cost-effectiveness of glimepiride and metformin treatment and to find out which treatment was cost effective and affordable at the Dr. Central General Hospital. Hasan Sadikin Bandung. This study began by taking samples from the medical records of patients with type 2 diabetes mellitus as a whole at the Hasan Sadikin General Hospital in Bandung and taking details of the cost of therapy. Samples that met the inclusion criteria were 105 patients and the clinical outcome was determined by the parameter of decreasing the patient's blood pressure. The analysis was carried out by calculating the effectiveness, ACER (Average Cost Effectiveness Ratio), and using SPSS (Statistical Package for the Social Sciences). The results of the analysis showed that the effectiveness of metformin was greater, namely 90.79%, while the effectiveness of glimepiride was 89.66%. The average cost of Metformin treatment is Rp. 14,296,678.22 while the average cost of Glimepirid treatment is Rp. 17,638,159.08. Metformin is more cost effective with ACER worth Rp. 157,469.75 while Glimeirid worth ACER is Rp. 196,722.72. Based on the test performed using SPSS, namely the Mann Whitney test, the result was 0.946 (≥0.05) so that there was no significant difference in the price of oral antidiabetics.
