Abstract
Indonesian Case Base Group was established by the Ministry of Health of the Republic of Indonesia which was used as tariff system of national health insurance (JKN) package. The rate of INA CBGs tariffs sometimes do not correspond to the real costs incurred by the hospital results the difference. The research purposes were determined the difference between real cost to the INA CBGs tariff, component that influence real cost, factor influencing real cost, and determined the real cost of chemotherapy related to the regularity of chemotherapy cycle in cervical cancer patient in Dr. Moewardi General Hospital of Surakarta in 2016. This is an observational research with cross sectional research design according to hospital perspective. Data was taken retrospectively from the medical record and the claims file of hospitalization service of cervical cancer chemotherapy patient. Data analyzing was used one sample t-test compare the real cost to the INA-CBGs tariff. The results showed there were 141 episodes of treatment from 46 patients where 51-60 years old is the highest age of cervical cancer patients. There is a significant difference between real cost and INA CBG tariff with differences that are on C-4-13-I treatment class 1 is Rp. -2.575.673, treatment class 2 is Rp. - 12.856.452, treatment class 3 is Rp. -18.219.951. In C-4-13-II treatment class 1 is 67.424.136, treatment class 2 is Rp. 15.715.007, treatment class 3 is Rp. 35 014.893, and at C-4-13-III treatment class 1 is Rp. 15.481.170, treatment class 2 is Rp. 5.220.245, and treatment class 3 is Rp. 3.022.613. Pharmaceutical costs are the highest cost component of the real cost. The treatment length at the severity level II is a factor affecting the real cost. The irregular cycle pattern has a larger average real cost than the regular cycle pattern of chemotherapy. Keywords: National Health Insurance, INA CBGs, cervical cancer, chemotherapy cycle
