Abstract
Diabetes mellitus was high cost disease, especially long-term complication treatment. Long-term complication treatmen cost was problem for patient, it can affect patients quality of life stadeted with utility value. Aim. The purpose of this study was to determine medical cost, utility value and leverage factors of diabetics outpatient. Method. This study was cross sectional design, data collected from retospektive medical record, finalcial depertement and pharmacy to obtain direct medical cost, utility value take from EQ-5D-5L questionnaire. Data analysed with mann-whiney and kruskal-wallis test. Results. The results of this study showed that medication for diabetics outpatient were biguanid, sulfonylurea, tiazolidindion, α-glucosidase inhibitor and insulin. Direct medical cost were pharmacy (Rp.352.940,00), laboratory (Rp.67.788,00), administrative cost (Rp.12.000,00) and services (Rp.1.000,00). Total amount was Rp.433.728,00 with biggest proportion was pharmacy. Result from EQ-5D-5L questionnaire showed biggest proportion on each dimension were 61% no problem on mobility dimension, 89 % no problems on self-care dimension, 54% slight problems on usual activities dimension, 41 % moderate problems on pain/discomfort dimension and 48% moderate problems on anxienty/depresion dimension. Build upon thailand value set, utility value was 0,833. Canlusion. Direct medical cost was Rp. 433.728,00 with leverage factor were pattern therapy, blood glucose level and complication.
