Abstract

Medication errors were often found in Ibn Sina Makassar hospital, one of them is writing a prescrition by a doctor obscurity. Therefore, researcher is interested in doing research in order to know the extent to which the incidence of medication errors in patients Jamkesmas Ibnu Sina Hospital in Makassar using Delphi and method to determine the factors causing the occurrence of medication errors by passing the FGD (focus group discussion) with the relevant parties. Research is a descriptive with technical data through direct observation of patients in peresepan the patient hospitalization jamkesmas RS Ibnu Sina in Makassar done in november 2011 2013 where instrument used in this research are a questionnaire with the analysis method to Delphi to identify the medication error that may be imposed on RS Ibnu Sina Makassar, check list of observation, pieces of data, and guide the group aimed ( focus group discussion ).The categories inclusion in these studies, the patients treated in the room was hospitalized jamkesmas, old treated patients at least three days and the status of the patient the criteria ekslusi is forcibly - home patients died. Data then proceed with the directional keompok ( focus group discussion ). Analysis of the results of research conducted using descriptive percentage. Based on the results obtained that the medication error that has an medication error the patient hospitalization Jamkesmas ibnu Sina Makassar in RS is a phase of dispensing error 215 occurrence ( 44,51 % ) followed the prescribing error is of 204 occurrence ( 42,23 % ) then there is the lowest figures in the administration error attention is as much as 64 ( 13,25 %) genesis meanwhile, the causes of medication attention to the patient hospitalization jamkesmas RS ibnu Sina in Makassar is, the preparation of the medicine workplace, the complete facilities factor the patient, lack of human resources, and the inadequate.
