Abstract
Routine hemodialysis patients in Indonesia continue to increase from 2013 into nowaday. The number of treatment required cause increase the risk of Drug Related Problems (DRPs) to affect outcome of therapy. The purpose of this study was to determine the characteristic of patients, treatment profiles in chronic renal failure patients with hemodialysis, case of DRPs in chronic renal failure patients with hemodialysis. The research method used were cross-sectional and retrospective retrieval method. The population of this study were patients with diagnosis of chronic renal failure with hemodialysis at Inpatient Installation of Dr. Moewardi Hospital Surakata in 2016. The sample of this study was medical record data of chronic renal failure patients with hemodialysis in 2016 in accordance with inclusion criteria. The results showed that from 73 patients, there were 10 cases of DRPs. The highest incidence of DRPs was dose of drug interaction 0%, dose too low 13.70%, dose to high 0% and drug without indication as 0% of cases. From the results of this study, it could be concluded that the number of occurrences drug related problems that occur affect achievement of creatinine reduction target, achievement of urea reduction target and length of stay (LOS) of patients with improved clinical outcome.
