One classification is type 2 diabetes mellitus in this type of diabetes, a decrease in the ability of insulin to work in peripheral tissues (insulin resistance and ß-cell dysfunction). Patients with type 2 diabetes are found in old age (geriatrics) and often have no previous symptoms. Antidiabetic drugs such as insulin and the OHO group are the therapies used for the treatment of diabetes mellitus. The purpose of this study is to determine the rationality of the use of antidiabetic. The research method used is descriptive retrospective with purposive sampling method. The sample of this research is medical record data of Diebetes melitus type 2 geriatric patient in January-December 2015 which adjusted inclusion criteria. The rationality analysis of this study using 4T principles is precisely the patient, precise indication, proper drug and appropriate dosage in accordance with the PERKENI 2015. The result of rationality of antidiabetic usage based on hospital formulary was obtained by 100%. According to PERKENI 2015, the patient is accurate 94,29%, exact indication equal to 98,57%, right of drug equal to 90%, and right dose equal to 77,14%.
