Abstract

Diabetic nephropathy is a chronic disease which is the main causes of kidney failure and highest mortality of all complications of diabetes mellitus. Over 20-40% of people with diabetes will suffer from diabetic nephropathy and many patients were unreachable by health services particularly in rural regions. Purpose of this study was to analyze cost of therapy in diabetic nephropathy inpatient, to discover differences between real costs against Indonesia-Case Base Groups (INA-CBG's) rates and to determine factors that affect cost of therapy in diabetic nephropathy inpatient. This study was observational analytic with cross-sectional design and utilize hospital perspective. Subjects of this study were diabetic nephropathy patient who were hospitalized in 2018. Data collected retrospectively. Data from this study were medical record data and Badan Penyelenggara Jaminan Sosial (BPJS) claim for diabetic nephropathy patients. Suitability analysis of real cost against INA-CBG's using one sample t-test, while the analysis of factors that affect real costs used bivariate correlation test. Result of this study showed that average cost for diabetic nephropathy therapy at Ir. Soekarno Sukoharjo Public Hospital in 2018 class 1, 2 and 3 were Rp.5.557.875, Rp.4.327.216 and Rp.4.225.696 respectively. There were differences in real costs of setting INA-CBG’s rates in treatment class 2, severity level I, grade 3, severity level I and class 3, and severity level III. The difference in cost of therapy with INA-CBG rates in 61 patients amounted to Rp. -11.888.021. Factors that affect real cost of diabetic nephropathy are Length of Stay (LOS), treatment class and disease severity. Keywords: Diabetic nephropathy, INA-CBG’s, Cost Analysis, Ir.
