Abstract

Hypertension can increase the risk of cardiovascular and kidney disease for diabetic patients. Kidney function impairment affect the drugs pharmacokinetic process, such as absorption, distribution, metabolism, and excretion. Proper dosage and drug choice can minimize toxicity risk and kidney function reduction. The aim of this study was to ascertain the accuracy of dosage regimen and drugs choice of antihypertensive in hypertension with diabetes mellitus patients at Bethesda hospital in 2017-2018. This study was descriptive non-experimental, all data collected with retrospective method. The sample that has been used is from the patient’s medical record who diagnosed with hypertension and diabetes mellitus, received antihypertension treatment, and hospitalized in Bethesda Hospital Yogyakarta in 2017-2018. To see the accuracy of drug selection and dosage compared to the JNC 7, JNC 8, and Drugs Information Handbook (DIH) guidelines. The obtained results showed that out of 30 cases, the most commonly used antihypertensive were Amlodipine (30%) as immunotherapy and combination therapy of Irbesartan with Amlodipine (10%). The antihypertensive evaluation drugs usage was found to be (100%) right drug and (86,7%) right dose. Inappropriate doses were found under dose (6,7%) and over dose (6,7%).
